Privacy Collection Statement – Activity Booking Request

Information collected

Greenacres Disability Services collects personal information when you make an Activity Booking Request. This includes but is not limited to:

Participant First Name and Last Name
Carer/Parent contact First Name and Last Name
Contact email Address and Phone Number
Information that you provide to us in special instructions related to activities.
We may also seek further personal and health information in relation to your support needs relevant to ensuring safe and appropriate support.
Other information about NDIS funding status, transport and equipment requirements.
We may also seek further personal and health information in relation to your support needs and available NDIS funding to provide the support.
We may also ask you to provide consent for us to speak to third-parties in relation to your support needs and available NDIS funding.

Your information is collected in order to:

Understand your support requirements.
Assess whether we can provide support to you to participate in an activity or activities.
Assess whether you have sufficient funding for Greenacres to provide the support to you.
Contact you if further information is required in relation to your activity booking request.
Conduct surveys, market research and analysis to provide services that meet your goals and support requirements.

If you do not provide this information, we will not be able to assess your support requirements and available NDIS funding for the activity booking request and you will not be able to participate in the activity.

Authorisation to collect information

Greenacres Disability Services is a registered NDIS provider. We are authorised to collect this information under the National Disability Insurance Scheme Act 2013 and the National Disability Insurance Scheme (Provider Registration and Practice Standards) Rules 2018.

You can access your information

You are entitled to access the personal information held about you by Greenacre Disability Services by speaking to our Engagement Office at customerservice@greenacres.net.au or contacting the Privacy Officer at privacy@greenacres.net.au.

Who do we share your information with?

We store your personal information securely and will not share this information with any other party, except where you authorise us to provide it to third-parties, as required or authorised by law or as stated in our Privacy Policy.
We are unlikely to disclose your personal information to services outside of Australia.
